Medication Guidelines

1. In order for the nurse to give out medications, student must have a prescriber/parents authorization (PPA) form filled out by the doctor.

- PPA form needs signatures from both the doctor and parent.

2. Over the counter medications must be new and unopened.

3. PPA form and prescription bottle must match.
